Sick Note / Fit To Work Note
If you have been ill for less than 7 days please read section on Self Certification.
Please note you will need to self-certify for the first 7 days, if you need a sick note beyond this period please complete the sick note request and submit proof of self-certification of first 7 days.
If you have been ill for more than 7 days your request will be assessed and reviewed by the doctor.
In certain circumstances, doctors may be able to backdate your certificate to a date from the start of your illness.
If you have been an in-patient in hospital, please request a certificate from the ward sister or nurse for a sick note for the duration you were an in-patient. Your GP is unable to certify periods of illness when you are admitted to the hospital.
Your GP will be able to issue sick notes for an extended period after discharge from the hospital.
Other Medical Certificates
We are often requested to write certificates, reports or statements in support of our patients for other areas in their lives other than employment. Examples: fitness to use a gym, fitness to fly, insurance reports, medical factors that might affect examinations, etc. We are happy to do provide these following a telephone consultation with a GP. Please ring the surgery to book a telephone consultation.
